Pacific Retirement Services Assisted Living/Memory Support Nurse Manager in Fort Worth, Texas

We are currently seeking an Assisted Living/Memory Support Nurse Manager who will work in coordination with the Assisted Living/Memory Support Administrator to direct and supervise the Trinity Terrace Assisted Living and Memory Care areas consistent with applicable state and federal law. Work Schedule: Monday-Friday, 8:30 a.m.-5:30 p.m. Hours may vary based on needs of the community. This position involves on-call.

  • Supervises employees, participates in personnel functions and works with Assisted Living/Memory Support Administrator in staff scheduling, training, assigning work, approaching overtime, hiring, terminations, performance evaluations and disciplinary actions.

  • Treats staff equitably, promotes a safe and harmonious workplace and works to reduce staff injuries

  • Remains current and compliant with Assisted Living State and Federal rules and regulations.

  • Implements policies and procedures to assure that the care, health and safety needs of the residents are being met

  • Performs and completes Evaluations and Service Plans in a timely manner. Comprehensive evaluations and service plans to be completed within 14 days of move in, annually and when resident has change of condition.

  • Develops a working knowledge of the special needs of those with dementia/Alzheimer’s. Understands appropriate programming and daily routines to assist in enhancing resident quality of life and care

  • Shall attend mandatory in-service training and be aware of changes in health and safety regulations, changes in employee policies and emergency procedures

  • Check 24 hour report, and follow up as needed

  • Check Alert Charting make sure charting is done for the day, and follow up as needed

  • Assessments: Comprehensive, and Service Plans: must be completed within 14 days of move in, Annually, and Change of Condition

  • All Admissions Paperwork including: Admission Checklist, Emergency Transfer and Door Packets, Medication Profile, Resident Care Directive, Skin Check, Complete VS and Weight, All About Me, Chart Order

  • Medication Management: Physicians Orders, Medication Room and Cart Storage (Including Ref), Dc’d Medications and Narcotic, Ordering and Reordering of all medications, Self-Medication evaluation, Observing Med Pass and CMA, Cart Audits, Check EMAR make sure all documentation is completed, and all other medication related items.

  • All Lab, X-Ray, and any test: Order, document, and follow up as needed

  • Physician Communication Book filled out and given to Physician when they are here at the community.

  • Acute Nursing needs for all residents including change of condition notification, charting, ect.

  • Incident Reports: Documentation, Investigation, Notification, Enter in AOD

  • Monthly Vitals and Weights: tracking of weight loss/gain and follow up as needed

  • Other Duties

  • Current licensure as a LVN in the state of Texas is required.

  • Must have a minimum of 2 years LVN experience in a long term care or related field. Requires a minimum of 1 year experience as a Nurse Manager. Previous experience in an Assisted Living and/or Memory Support environment is highly preferred.

  • Must possess (or in a timely fashion) obtain a working knowledge of state and federal regulations regarding the operation of a health care facility as well as OSHA regulations.

  • Must be insurable under the company insurance carrier

  • Role requires substantive interaction with staff in other departments, residents, families and volunteers. Requires a high level of tact, persuasion, judgment, integrity, and timing.
